Managed Services Agreements

Use the agreement for your customer type with the applicable Service Order and schedules. The Cancellation and Refund Policy Amendment accompanies both agreements. Consumer agreements also include the Consumer Cancellation Notice and its two cancellation forms.

These are blank forms. Customer details, selected services, prices, dates, and required signatures must be completed for the individual transaction. Publishing these forms does not change an existing signed agreement.
